Life Health and Wellness Health Care Medical Clinic began in Phoenix in 2003 to serve the health care needs of the Community. Our Veteran contracts allow us to serve Veterans, Active duty Military, Reservist and their families. We also serve our community, any persons in need of health care. Our current location is 6605 N 19th Avenue in Phoenix. This location has two health care providers. Life Health and Wellness Health Care providers also manages the Wellness Center at the Phoenix Dream Center located at 3210 NW Grand ave in Phoenix. All services are provided at each facility. Our facility provides the best care possible while maintaining HIPAA (Health Insurance Portability and Accessibility Act) compliance for our patient’s electronic health records.
